Stripe is the tool used whenever an account transaction needs to be checked and/or updated. Examples of these Stripe functions are refunds.

It is a must that customer support agents leave notes whenever changes or updates are made using the Stripe tool.
Whenever a refund is made, there will be a pop-up that will ask for the amount and details about the refund. Customer Support Agents are expected to leave notes that can be as brief as just the Ticket Number and two words like "Unintended Renewal", or a summary of why the change was made such as "Claims for 30 days refund, provided prorated refund due to completed courses":
In case the change in Stripe did not trigger a pop-up for the details, like canceling a subscription with a refund. An Add note button can be found when the affected transaction is clicked:
